Welcome !!
Open to residents of Pike County, we have (3) three different programs available.
All (3) three applications for Transportation are listed here, Shared Ride (SR), Medical Assistance Transportation Program (MATP), and People with Disabilities (PwD).
We hope they will help you with your transportation needs.
Please choose the program that best suit you for a application:
** All applications require verification. **
** all appointments must be given at least 48 hours in advance **
Shared Ride Program (SR)- Available for seniors 65 years and older.
Please print application, fill out and mail to:

Medical Assistance Transportation Program (MATP)- Available for consumers who have a Access Card
Please print out both of these forms; MATP Application - Eligibility form
People with Disabilities (PwD) - Available for consumers who are disabled.
